We assess your home environment and evaluate the individual's abilities and challenges from a physical, emotional, and cognitive perspective. This allows us to provide suggestions for immediate, short-term, and long-term solutions to address fall risk hazards and aging in place. This can include equipment prescription, when necessary.
A mobility assessment can be valuable to individuals whose mobility has been impaired and/or has a higher risk of falls.
Mobility assessments are typically used to support application for funding sources such as the March of Dimes. Following the assessment a supportive letter is provided outlining current abilities and challenges, as well as recommendations.
When an individual is ready to be released from acute or in-patient rehabilitation, discharge planning helps to ensure their health care needs and safety are identified and prepared for when they return to their place of residence.
This process will include a Home Safety & Functional Assessment, and may involve the hospital team members, family members, community health care providers, etc.
We understand the importance of caring for yourself and having control and independence in your day-to-day life. We work collaboratively with our clients to help them return to meaningful and purposeful daily activities.
Treatment related services may include:
Activities of Daily Living re-training (e.g. dressing, feeding, hygiene, bathing, meal preparation, grocery shopping, etc.).
Cognitive training and rehabilitation (e.g. memory, concentration, procedural learning, executive functions, etc.).
Coping skills training (e.g. anxiety management).
Community integration training (e.g. exposure therapy; practicing with mobility aids, etc.).
Education about establishment of daily routine.
Education for family members and caregivers.
Fine motor and gross motor training.
Return to work or school planning.
Self-care practice.
Transfer practices (e.g. chair, bed, bathtub).
Writing skills training.
