Our team provides comprehensive discharge planning, functional assessments, and evidence-based treatment designed to restore independence, maximize function, and enhance overall quality of life. Because we are a mobile clinic, we bring therapy directly to our clients, at home, work, school, or within the community, ensuring convenience, accessibility, and personalized care.
We work with clients experiencing a wide range of challenges, including but not limited to:
- concussion and traumatic brain injury
- musculoskeletal and orthopedic injuries
- spinal cord injury
- amputation
- chronic pain
- and mental health concerns.
Our services also include home safety assessments and recommendations for home modifications to support safe and independent living.
Referrals are accepted from clients, physicians, hospital and rehabilitation teams, lawyers, and insurance representatives. We assist with completing all necessary insurance forms, including assessment and treatment plans (OCFs) and disability certificates, to streamline the process for both clients and referral sources.
